How To Register Your Child for Therapeutic Services

Call your child’s PCP and request that a referral be faxed to 207-888-2223 for an evaluation and therapy services at Bright Start Therapy.
02.
Once a referral has been received, the office adminstrator will call to set up an initial evaluation appointment with you and your child.
03.
Office manager will send you enrollment forms that must be completed prior to your appointment. Should you have questions regarding policies and procedures you may call the office.
04.
A masters level clinician will evaluate your child. The evaluation report will be faxed to the child’s PCP and email to you via a secured portal.
05.
Should your child require direct therapy services; the office administrator will reach out to you to set up weekly therapy appointments.
